PASSPORT FOR A CHILD
If applicant is 16 or 17 years old: One parent must be present
If applicant is 15 years old or younger: Both parents must be present.
Parents must have a valid state issued driver’s license, I.D card or passport.
A child's passport is not renewable.
PASSPORT NAME CHANGE
If the name you are currently using is different from the name on your most recent U.S passport, you must provide a certified copy of the legal name change document.
You can renew your passport only if it's undamaged and
all of the following apply:
- It has been expired for less than 5 years
- You were 16 or older when you got it
- It's in your current name (or you can legally document the name change)
If your passport has been significantly damaged, especially the book cover or the page displaying your personal data and photo, you will need to apply for a new passport. Damage that might require you to replace your passport includes water damage, a significant tear, unofficial markings on the data page, missing visa pages (torn out), a hole punch, or other injuries.
Frequent travelers who do not have enough time to obtain a visa while needing to travel to a different country can apply for a second passport. This is a limited-validity book valid for 4 years.
